We have a modern building developed
especially for the practice of Oral & Maxillofacial Surgery. It is spacious
and beautifully landscaped and has many large exterior windows and skylights
for the use of natural light. Located close to one of the most active
intersections in this area, it is easily accessible.

- Our office hours are as follows:
- Monday: 8:30 AM to 5:00 PM
- Tuesday: 8:30 AM to 5:00 PM
- Wednesday: 8:30 AM to 5:00 PM
- Thursday: Cosmetic Surgery by Appointment
- Friday: 8:30 AM to 5:00 PM
- General anesthetics are administered on Monday, Tuesday and Friday.
Financial Policy: - We are providers for many insurance plans but not all insurance plans.
- You can obtain a list of providers from your plan, or you may call
our office and we can usually tell you if we can provide your care.
We accept Visa and Mastercharge and Discover cards.
We accept Medicare for covered procedures.
- Payment for service is as follows:
- Full payment if there is no insurance or for non-covered procedures
- We will require a percentage of the total fee plus the deductible (if not met) with some participating insurance plans. Otherwise, we will require payment of the first $350 for any charges over $350.
- Co-pay at time of service for HMO patients.
- Full payment for cosmetic procedures must be made 2 weeks prior to appointment
- You must obtain a medical referral from your plan, if required, for
charges to be paid by the HMO or PPO.
Minors and dependants: - A responsible adult with custody must accompany any minor to receive
care and remain in the office during treatment. Prior written consent
must be in our possession for minors to be treated without a parent
present.
- A responsible adult must accompany any minor for surgical care.
